Appointments:   Monday 9am-5pm,  Tuesday Noon - 8pm,  Wednesday 9am - 6pm

         Thursday 7:30AM - 1pm,  Friday 9-1pm (reception only),  and e/o Saturday 8:00 -3:30pm

   Estimates:  available in writing.

   Insurance:  Most insurances accepted. You have no forms to complete. Claims are           

        sent same-day, electronically.  Maria  estimates**   your insurance benefit and asks you

        to pay the difference at your appointment.   

  ** Our computer system tracks hundreds of insurance plans. This is good  faith estimate. Dr. Denholtz renders a personal service to you and you are ultimately responsible for its cost. Some insurance plans tell participants they "will be covered 100%", but do not clearly explain, deductibles, annual maximums or other limitations.

   Payment Options:    1) Check, cash, credit or debit card;  

                                         2) 5% Pre-pay discount; 

                                         3) 12 months interest free  (with Care Credit - subject to credit approval); 

                                         4)  Monthly charge to your credit card.
